TFPD Emergency Medical Information (EMI) Program

The Tallahassee Fire Protection District (TFPD) Board of Directors has launched the TFPD Emergency Medical Information (EMI) Program.

What is the TFPD EMI Program?

The EMI Program is a voluntary, community-wide initiative designed to help TFPD First Responders quickly access vital medical history, support needs, and emergency contact information during an emergency. This information is stored in a completed EMI Form placed inside a clear plastic bag on your refrigerator door. Every second counts — this simple step could save your life!

Why participate?

Imagine taking a nasty fall at home. Your spouse calls 911. When TFPD First Responders arrive, they need to know your medical history before treatment. In the panic of the moment, neither of you can recall essential information. Valuable time is lost.

Now imagine the same event — but this time, responders see the TFPD EMI Decal on your door. It tells them you’re a program participant. They immediately retrieve your EMI Form from the refrigerator and can begin treatment with knowledge of your medications, allergies, and conditions. No time is lost — help arrives informed.

What you’ll receive

You’ll receive a plastic baggie packet with a TFPD EMI Decal on the front. Inside, you’ll find:

  • One EMI Decal (to apply near your entry door)
  • One “Why + Instructions” sheet
  • Two EMI Forms

What to do

Follow these steps to complete the program setup:

  1. Complete the EMI Form with accurate, up-to-date medical information.
  2. Place the completed form(s) back into the plastic baggie.
  3. Securely place the baggie on your refrigerator door.
  4. Apply the EMI Decal near the most-used entry door — at eye level and within three feet of the door.
  5. Ensure surfaces are clean and above 55°F before applying the decal. Avoid rough or dusty surfaces.

Program cost

Completely free! The TFPD fully sponsors the EMI Program — there is no cost to participants.

Packets are distributed at community events, HOA/POA meetings, and at the Main Station. You may also request delivery:
